Please fill out this form. Every applicant MUST fill this form out in order to be considered by Bridge Play Foundation Grants Committee.

The (teams) application fee for the Game Changer Grant Program is $15. 100% of the funds collected are allocated directly to the Game Changer Grant Fund.

Documentation of Need: Attach any relevant documentation supporting your request (e.g., quotes for uniforms, invoices for equipment, league registration costs).

Commitment Certification: By applying, you certify that if your team is awarded funds:

• You will display a banner at every game featuring Bridge Play Foundation’s logo and the logos of our partners.

• You agree to be listed on our website, social media platforms, and other promotional materials as a recipient of the grant.

• Failure to meet these requirements may result in disqualification from receiving future funding.
