Program Location: James Monroe High School

Program Time: 6:00 PM – 8:00 PM

Days: Wednesdays and/or Fridays

Program End Date: March 13, 2026

Eligible Ages: 14–17 years old (High School Students)

Youth Participant Information

Participation Permission

I give permission for my child listed above to participate in the Bridge Play Foundation Winter Batting Clinics held at James Monroe High School from 6:00 PM to 8:00 PM on (check one):

Media Consent

I give permission for my child to be photographed or recorded during program activities. These images or videos may be used by Bridge Play Foundation Inc. for promotional, educational, or social media purposes.

Dismissal Acknowledgment

I understand that my child will be leaving the program on their own at dismissal time (8:00 PM). Bridge Play Foundation is not responsible for transportation after program dismissal.

Release & Liability Waiver

I understand that participation in athletic activities involves inherent risks. I voluntarily release and hold harmless Bridge Play Foundation Inc., its staff, volunteers, coaches, partners, and James Monroe High School from any liability for injury, accident, or loss that may occur during participation, except in cases of gross negligence.

I confirm that my child is physically able to participate in this program.

Youth Code of Conduct (To Be Signed by Participant)

I agree to:

• Follow all program rules and staff instructions

• Treat coaches, staff, and other participants with respect

• Use appropriate language and behavior at all times

• Take care of equipment and facilities

Failure to follow the code of conduct may result in removal from the program.
